Почитай README и help. Там какой-то конфиг в проге нужно отредактировать.
Ни в хелпе (там сказано только, что Рефман инсталирует свои тулзы в Ворд и все ни в readme нет ничего подобного.
Раньше все было в порядке (правда сейчас не помню был у меня уже тогда Офис ХР или нет м.б. придется пробовать на более старой версии офиса, хотя очень не хотелось

Говорили, что 11 Рефман якобы криво взломан, а у меня 10 лицензионный
А с EndNote не работала никогда, да и база у меня уже Рефмановская, не хочется с нуля создавать новую

Только что в хелпе нашел (troubleshooting):
No Reference Manager Toolbar or Commands in Microsoft Word
When you install Reference Manager 11, Cite While You Write (CWYW) tools are installed in Microsoft
Word for the current user account only. For example, if you are logged in to Windows under the
Administrator account, the Reference Manager tools in Word install only for the Administrator’s profile
and not for any other user accounts. If you log in to Windows under a different user account and run
Microsoft Word, you will not see the Reference Manager tools in Word under the Tools menu, nor will
you see the Reference Manager 11 toolbar. If you do not see the tools, you can try one of the following:
· In the Reference Manager program folder, locate the file RefManConfig (or RefManConfig.EXE,
depending on your Windows settings). With all other programs closed, doubleclick or Run this file. A
“Configure Reference Manager” window appears. Leave the “
Cite While You Write for Microsoft Word 2000, XP, or 2003” box checked and uncheck the other two
boxes. Then click Next.
· Run the Reference Manager 11 Installer again. If the program has already been installed on this
computer, when you run the installer again you will be given the choice to “Repair” or “Remove.” Select
"Repair” and proceed with the installation.
· If you cannot try the first two options or they do not work when you do, you can manually install
the CWYW tools into Word as follows.
To copy the required files to your Word Startup folder:
1. Within Microsoft Word, select Tools>Options.
2. Click the File Locations tab, highlight the Startup folder, and click the Modify button.
3. Note the path to the Microsoft Word Startup folder. If no path is listed at the bottom of the window
click on the “Look In” dropdown list at the top of the window and note the path.
4. Close Microsoft Word (and Outlook if running).
5. Launch Windows Explorer and locate your Reference Manager 11 program folder. The default
installation path is C:\Program Files\Reference Manager 11.
6. In the Reference Manager program folder, open the CWYW folder.
7. There are two files in this folder you need to copy:
RM11CWYW.dot
RM11CWYW.wll
If you do not see the .wll and .dot file extensions, go to the Tools menu and select Folder Options>
View tab. Uncheck the box that reads “Hide file extensions for known file types” and click OK.
8. Locate the Word Startup folder (that you identified earlier) and paste these two files into the folder.
If you cannot find the specified folder, your Windows preferences may be set to hide the folder. In
Windows Explorer go to Tools >Folder Options and click on the View tab to make sure that "Show
hidden files or folders" is selected.
9. Start Reference Manager, open a database, and then open Microsoft Word.
Reference Manager 11 commands should now appear on the Tools menu in Word and on a Reference
Manager toolbar.
Правда это для 11, но для твоей версии подойдет. Или посмотри в соответсвующем разделе у себя.

да и база у меня уже Рефмановская, не хочется с нуля создавать новуюПри желании можно и конвертнуть.
Кстати у меня была похожая проблема с RM11/EndNote9 (но причина, видимо, другая) - менюшки принципиально не работали в исходном Word XP. Но после установки на него любых sp всё работает.
У меня на офисе оба сервис пака стоят.
Конвернуть-то можно, только у меня уже половина ссылок в диссере Рефманом вставлены,насколько я понимаю, их пришлось бы по-новой вставлять, а это лишнее время которого нет

Это не очень долго. В дипломе на 90 стр. я 210 ссылок расставил за 2 часа.. 
Мне больше понравился вариант за 5 минут поправить конфиги

Рефман 10 не хочет инсталлировать свою менюшку в Ворд ХРМ.б. кто -нибудь знает как с этим бороться?
